GLP-1 medications work well regardless of your age, but if you're over 40, there are a few things worth paying extra attention to. The big one: age-related muscle loss (sarcopenia) is already happening naturally, and rapid weight loss can speed it up. That doesn't mean GLP-1s are a bad idea after 40, it means you need a smarter approach.
The Muscle Loss Concern
Research shows that 25-40% of the weight people lose on GLP-1 medications can be lean mass, muscle and bone, not just fat. For someone in their 20s or 30s, that muscle usually comes back without much effort. After 40, it's a different story. Rebuilding muscle gets harder with each passing year, and losing too much affects your mobility, balance, and metabolism.
Protecting Muscle Mass
Resistance training isn't optional here. Two to three sessions a week targeting major muscle groups makes a real difference. You don't need a gym membership, bodyweight squats, resistance bands, or a pair of dumbbells at home will do the job. What matters is gradually increasing the challenge over time.
Protein needs go up, too. Older adults need more protein per meal to trigger muscle protein synthesis than younger people do. Ask your provider or a registered dietitian for a protein target based on your weight and kidney health, and spread it across all your meals instead of loading up at dinner.
Bone Density Considerations
Rapid weight loss at any age can thin your bones. For women over 40, especially after menopause, that compounds an already elevated osteoporosis risk. Talk to your provider about a bone density screening before starting GLP-1 therapy, and make sure you're getting enough calcium and vitamin D.
Metabolic Advantages
There's an upside to being in this age group, though. GLP-1 medications improve insulin sensitivity (learn about common GLP-1 side effects), which matters more as insulin resistance tends to creep up with age. Some of these drugs can also protect the heart. Wegovy is FDA-approved to lower the risk of heart attack, stroke and heart-related death in adults with established heart disease and obesity or overweight. Ozempic has that approval for adults with type 2 diabetes and established heart disease, and Mounjaro for adults with type 2 diabetes who are at high risk of those events. Semaglutide has also been tested for brain protection, but in 2025 two large trials in early Alzheimer's disease did not show that it slowed the disease.
Realistic Expectations
Weight loss after 40 might be a bit slower than what younger patients see. Honestly, that's a good thing. Losing 1-2 pounds per week gives your body time to adapt, helps you hold on to more muscle, and tends to stick better long-term. Steady and sustainable beats dramatic any day.
Working with Your Provider
Find someone who does more than write a prescription. The right provider monitors your body composition, not just the number on the scale, orders regular lab work, and coordinates with your primary care doctor if needed.
