- Insurance-negotiated rates and copay amounts
- Prices below $50/mo or above $1,500/mo (likely data errors or outliers)
- Clinics without publicly listed pricing
How we calculated
- Median (not mean) used to reduce impact of outliers
- 25th and 75th percentiles represent the typical price range
- State-level statistics require at least 3 data points
- Retail pharmacy prices sourced from manufacturer list prices
