Zepbound Cost, Savings Programs, and Insurance Coverage (2026 Guide)

Zepbound (tirzepatide) is the newest FDA-approved GLP-1 medication built specifically for weight loss. It's Eli Lilly's answer to Wegovy, and the clinical results have been strong — but cost is still the biggest hurdle for most patients.

Retail Price

Without insurance, Zepbound's list price sits around $1,060 per month. What you actually pay at the pharmacy can vary depending on your dose and where you fill it.

Eli Lilly Savings Card

Eli Lilly runs a savings program for eligible patients. If you have commercial insurance that covers Zepbound, you could pay as little as $25 per fill. Even without coverage, the LillyDirect program offers reduced cash-pay prices. Check Lilly's official site for the latest details — these programs change often.

Insurance Coverage

Coverage for Zepbound is growing, but it's inconsistent. Most plans require prior authorization — you'll typically need to show a BMI of 30+ (or 27+ with a weight-related condition), documented failed dietary interventions, and sometimes a prior trial of a different weight loss medication. Medicare still doesn't cover weight loss drugs, though there's legislation in the works.

Telehealth Options

Plenty of telehealth clinics now prescribe Zepbound or compounded tirzepatide at bundled monthly rates. These packages usually include consultations, lab monitoring, and the medication for $350-$600 per month. Compare what's available in San Antonio and San Jose.

Zepbound vs Mounjaro

They're the same active ingredient — tirzepatide (see our Mounjaro cost guide). The difference is labeling: Zepbound is FDA-approved for weight loss, Mounjaro for type 2 diabetes. Our Zepbound vs Mounjaro comparison breaks it down further. Which one gets prescribed often comes down to your insurance. Some providers will prescribe Mounjaro off-label for weight loss if the coverage is better that way.
